Scholarship Application

Eligibility
Each year CWA Local 9586 awards four $500 scholarships. Applicants must be members or their children. You must be a member of Local 9586 on April 7, 2004 to be eligible. Employees of VERIZON will only be eligible if their course of study is NOT covered by the company's tuition aid program.

In the fall a check for $250 will be mailed to the winner's chosen college or university's financial aid department where registration will be verified and the funds dispersed. For the spring semester,k the same procedure will be followed except that the Local will first verify completion of at least two classes in the preceding semester or quarter before payment will be sent to the school. In no case will the funds be dispersed directly to the winner.

Instructions
Please fill out Section A completely and return to the Local. When your application is received, current membership status in Local 9586 will be verified and your application accepted.

There are also scholarships on a National level our members and their family may be eligible for. For more information please contact Gregg W. Gibson or Judy Rapue.

Drawing
Winners are selected by a lottery type drawing at the April 7, 2004 membership meeting, which is held at 7 p.m. at the Local. Applicants and their parents are encouraged to attend.

The names of the winners will be put on the Local recorder the evening of the membership meeting.

ONLY THE WINNERS WILL BE NOTIFIED OF THE RESULTS OF THE DRAWING.
