FMLA~
It is the
purpose of the Act to balance the demands of the workplace with the needs of
families, and to entitle employees to take a reasonable leave for medical
reasons, for the Birth or Adoption of a
child, and for the care of a child, spouse, or parent who has a serious health
condition.
If you have worked for Verizon for one year
and have a total of 1250 hrs. and have a Serious or Chronic health condition
you can apply for FMLA.

Chronic
serious health condition~ continues over an extended period of time, requires
periodic visits to the doctor for treatment, generally causes episodic periods
of incapacity
·
Long-term
or permanent disability~ Conditions causing long-term or permanent disability,
including terminal illnesses, are serious health conditions
·
Intermittent
Leave~ if a serious health condition incapacitates you on an occasional or
sporadic basis you may take FMLA intermittently.
·
If
you are out of work due to an illness that incapacitates you for three days or
more.
·
Pregnancy
~is a serious health condition. Medical leave may be taken for any period of
incapacity, including appointments for pre-natal care, severe morning sickness,
nausea, dizziness.
(Please note that FMLA runs concurrent with
Pregnancy Disability Leave and the 1250 BPO hrs. are not required to utilize
PDL)
When
applying for FMLA you should be aware that you have 25 days from the first day
of absence. These days are consecutive and include weekends. If you cannot
provide the FMLA form to the FMLA team by the required timeframe due to a delay
by your healthcare provider please make sure that your healthcare provider
indicates that it has been” delayed
through no fault of the patient.”
If
not then your form will not be considered due to the untimliness.
If
you have submitted a form and the FMLA team sends you a correspondence
indicating that you have an incomplete
form. You must get the required information that is being requested and
resubmit the form by the timeframe indicated on the correspondence.
Once
again if you cannot get the Form returned by the specified date requested by
FMLA, due to your healthcare provider, have your healthcare provider indicate “delayed through no fault of the patient”.
At
times you may be requested to do an Administrative Review for your request for
FMLA. What this means is that the FMLA team may need additional medical
information/ documentation to approve your request for FMLA, along with a brief
explanation as to why you feel you should be approved and by including the
supporting medical documentation you can expedite the appeals process.
Please
keep in mind these timelines~ they are quite important and if not followed
correctly your requests may, can & will be impacted.
